The Intake Coordinator, LPN is responsible for coordinating all new referrals/inquiries made to the agency, insuring that all new referrals meet the agency’s policies and procedures as well as federal/state regulations and guidelines. This position is located at the Madison, Wisconsin office. MAJOR RESPONSIBILITIES/JOB FUNCTION Clinical Coordinates directly with all referral sources via phone, fax or e-mail regarding new client referrals. Assures agency intake processes meet applicable local, state or federal licensing/regulatory requirements in addition to agency policies and procedures. Coordinates with agency RN Case Managers to assure efficient admission of new referrals. Coordinates with agency Service Coordinators/staffing Specialists insuring appropriate staffing coverage for new referrals. Assures that authorization for service is confirmed for all new referrals. Administrative Ensures effective and timely coordination of client home care services through the timely completion of required documentation and computer data entry for new intakes, as well as timely transfer of pertinent medical data to client’s physician, therapists and agency staff members. Provides input to the Director of Nursing regarding the need for formulation or modification of agency policies, procedures and practices pertaining to client intake and services. Maintains accurate and comprehensive client medical data throughout the intake process. Notifies Branch Manager regarding proposed changes that may affect intake practices. Notifies Branch Manager regarding problems in the service environment that may affect service delivery or client care. Other responsibilities or duties as assigned by the RN, Intake Coordinator or Branch Manager. Communication Communicates in a clear and professional manner. Demonstrates fairness in handling new referrals and in employee/client issues.
