The MSC is responsible for managing complex medical cases and health care needs across the continuum of care using the care management process to ensure optimal care, services, and provides care coordination for the WEA Trust (the Trust) members. The MSC effectively utilizes clinical criteria guidelines and accreditation standards as well as department processes and procedures to achieve utilization targets. ACCOUNTABILITIES: 90% - Optimal health care utilization for Trust members: Coordinate and manage complex medical cases and services provided to care management patients resulting in appropriate care and cost effective outcomes. Exhibit strong clinical judgment to ensure that the services being requested are medically necessary and/or medically appropriate utilizing established protocols, medical policies, and purchased clinical criteria guidelines. Gather relevant clinical information specific to the targeted medical or behavioral health condition and assess resource utilization and cost management, diagnosis, past and present treatments, and prognosis. Monitor the treatment plan to: Optimize patient outcomes to reduce costs and offer the least restrictive setting. Ensure appropriate and consistent care to reduce variation of care. Enable shared and informed decisions working collaboratively with the member and provider. Identify appropriate providers and facilities throughout the continuum of care while ensuring available resources are used in a timely, cost-effective manner and in accordance with established clinical practice guidelines. Identify and evaluate candidates for care coordination (discharge planning and transition of care). 10% - Research and development: Research and identify standards of care and contemporary medical practices. Assist in the development of effective internal clinical guidelines that assure appropriate and consistent coverage decisions and medical review standards, policies, and procedures. MAJOR AREAS OF ACCOUNTABILITY: Greets, directs and waits on customers. Answers phone calls in a professional, courteous manner. Writes up orders and rain checks, opens accounts, checks prices, rings up sales, makes change and authorizes credit card purchases. Counts down the cash register drawer by using intelliscale to verify funds, places the correct daily starting currency in the cash drawer and prepares daily deposit for bank. Offers Service Protection Plans (SPP’s) and other qualified programs as available and dictated by Company policy. Acquires supervisor’s approvals on all disbursements of Company funds, returns, and credit authorizations. Down-stocks, faces and fronts all merchandise in their assigned areas according to Company merchandising standards. Protects the store against theft or other suspicious activity. Follows Company approved Customer Service Standards. Keeps supervisor informed of important information developments, potential problems and related information necessary for effective management. Performs related work as apparent or assigned.
