Evaluate and educate staff and community members on current educational materials on bi-annual (staff) and annual (community) basis while insuring educational materials are available to staff and patients at all times. 4.) Establish and foster working relationships with community members, nursing staff, and physicians involved in the care of stroke patients. Provide and foster open communication between physician and other health care professions. 5.) Facilitates, supports, and co-leads with the medical director in the stroke team committee. 6.) Collects required data monthly and enters into the assigned database. In addition, prepares monthly reporting on program metrics using benchmark data and makes specific recommendations for improvement 7.) Handles all aspects of the Joint Commission re-certification process. Acts as lead facillitator for all Stroke program surveys and submits required data to the certifying body on a quarterly basis.
