The assignment of codes are based on CMS , CPT /AMA coding guidelines. The coder will adhere to the AHIMA Ethical Coding Standards. Concurrently reviews physician documentation (handwritten/dictation) to assign appropriate diagnoses and E/M codes each day. Reviews all encounters for quality/all elements of clean bill prior to release for billing. Cross training with other division(s) to ensure coverage and charge capture. Maintains, daily, a Medical Record List to ensure timely charge capture. Outpatient Coding: Reviews physician documentation (handwritten/dictation) to assign appropriate diagnoses and E/M codes for all encounters by date of service. The Professional Fee Analyst will maintain a lag time (date of pick up from clinic to final coding) of no more than 5 business days. Reviews all encounters for quality/all elements of clean bill prior to release for billing. Queries physicians when code assignments are not straightforward or documentation is inadequate, ambiguous, or unclear for coding purposes. Reviews and corrects pre-bill TES edits in assigned work file pertaining to coding and medical necessity issues, twice a week. Maintains records of work completed to ensure all work processed is timely and maintains a quality standard of no less than 98% accuracy. Details: Fuel all Straight trucks, tractors, pup trailers and refer units Wash all outside windows and mirrors Fill all window washer fluid in the trucks and keep them full Check tire pressure and all fluids (oil and coolant) Check trailers refers that are in the yard or indoors. ¼ or less needs to be filled Check to make sure all trucks have enough DEF in them Check reading from fuel storage tank daily Inform the garage and dispatch of any defects you find When needed to hook/unhook all tractors to/from the pup trailers Pick up any or all food service carts that are on the ground Keep the fuel island and the shack clean and neat at all times Keep all paperwork clean and accurate at all times* Follow Good Manufacturing Practices. Maintain a clean, sanitary and safe work area. Follow all required work safe practices. This would include wearing of all required PPE in designated areas, confined space safety, safe chemical handling, fall restraints, etc. Perform other duties as assigned.
