Details: CareHere is seeking to immediately employ a Clinical Care Coordinator (LPN/ RN) for our medical clinic in Rice Lake, WI. This will be a Part Time position working 13 hours per week. SUMMARY: The Clinical Care Coordinator/Disease Management Nurse will work onsite at the wellness center and be responsible for over-seeing patient preventive screening, gaps in care, wellness education, and internal and external referrals related to these activities. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Clinical Care Coordinator/Disease Management Nurse will review the schedule of patients approximately 24 hours prior to each center day. The EMR of each patient will be reviewed for gaps in care, needed preventive screening, follow up on internal and external referrals. Provider will review with patient and write any necessary orders. Responsible for documentation in the EMR patient encounters and maintain patient communication per established disease management protocols. Responsible for educating patients on how to access CareHere Connect and reinforce education on how to be compliant with specific employer incentives. Establish an on-going relationship with clinic staff to educate on wellness programs, process, and documentation for each. Assess patient need for scheduling with a particular coaching expertise. Assess need for referrals to both coaching staff and clinic service (i.e. time for annual HRA or follow up on lab results). Make appropriate referral and follow up on patient compliance. Neatly, accurately and legibly maintain all records and documentation. 9. Minimize overrun. 10. Document and adjust quantities required relative to the amount of overrun used. 11. Keep work area neat, clean and organized at all times. 12. Maintain communication with end of the line to assure overrun usage and filling the order correctly and completely. 13. Aware of the value of all necessary tools and equipment and maintains them for optimal use. 14. Positive attitude toward work and co-workers. 15. Able to work overtime. 16. They must also display confidence with a friendly and outgoing personality. Proficiency with computer data entry, typing, and transcription is a must. The position consists of four days a week, one of which is located in our Beaver Dam office.
