Details: About the Opportunity Unity is currently seeking a full-time nurse practitioner to join our medical team. The medical team is comprised of one medical director, three assistant medical directors, 10 medical advisors and five nurse practitioners. Assist in overseeing hospice and palliative care provided to patients residing in their homes, assisted living facilities, contracted hospitals and at Unity’s 12-bed hospice residence. This position is a part of a comprehensive continuum of care serving our hospice and palliative care patients. It involves consulting and collaborating with Unity’s medical director and assistant medical directors in addition to the patient’s attending physician.
