Details: This position is going to require an employee to take a schematic and wire up a panel or control that has already had all of the holes drilled out for them. They will have production goals to meet with this each week. They also provided a schematic test this candidate would need to take and pass before being considered. Must be a good team player, be on time everday and not miss work during this 3 months period, and be very detailed with work. The employees will receive 1 month of training before they are expected to fully meet the production goal which will change each day/week based on the order. This is not a job shop so the schematic will be basically the same that they are working off of. Their best employees are able to read from a schematic and perform wiring with great detail and very neat/clean panels. Provides consultation, problem solving, guidance and education to the hospice and home health teams as needed to assure optimal palliative care for patients and families. Engages in immediate, frequent collaboration with the Medical Director to share patient progress and plans of care, as well as active problem solving. Collaborates with the Attending Physician as needed to maintain an effective plan of care that produces excellent outcomes for the patient and family. Promote a positive culture among and across the hospice team. Promote teamwork and shared responsibility for attaining the agency mission and vision. Actively engage in QAPI initiatives and compliance activities. Take part in professional development to maintain qualifications for role; completes CBTs in a timely manner. Conducts face-to-face assessments for recertification and readmission as necessary. The Nurse Practitioner evaluates clinical evidence for eligibility and provides this information to the Medical Director and hospice team. While the Nurse Practitioner provides information and summary, the Medical Director makes the determination of and writes the certification/recertification of terminal illness. Assists the hospice team in the development of the comprehensive assessment of patients and families. Communicates and collaborates with the Medical Director and obtains their input in the care of patients/families. Occasionally provides medical services for hospice patients as directed by the hospice team when the primary provider is unavailable or wishes for Nurse Practitioner support. Provides input in the definition and measurement of meaningful outcomes that assess effectiveness of interventions and medications. Readily offers suggestions that promote continuous improvement of outcomes and the safe provision of care. Assures that all elements of performance meet the Medicare Hospice Conditions of Participation. Maintain productivity standards. Promote efficiency in use of supplies and visit utilization including mileage. Assures appropriate medical care to achieve clinical and financial outcomes. Is an ambassador for the agency in daily encounters with referral sources. Promote positive relationships with contracted facilities. Minimum Qualifications: Education/Licensing/Certification: Master's Degree in Nursing. Current state Registered Nurse licensure. Nurse Practitioner certified (or ability to sit for certification exam within three (3) months) through ANA or equivalent. Current state Advanced Practice Nurse Prescriber certified or eligible for certification. Possess valid driver's license, proof of insurance and access to automobile. Must be CPR certified. Experience: Five years nursing experience Previous experience working as an Advance Practice Nurse. Experience in Palliative Care and/or Hospice preferred.
