Details: JOB SUMMARY: Provide comprehensive personal care and assistance to facility’s residents under the direction of the nursing staff. Follows all orders of charge nurse and supervisor regarding resident care and cleanliness of unit. Responds to the facility’s commitment to service by establishing good rapport with families and visitors. DUTIES AND RESPONSIBILITIES: 1. Provides complete care of each resident in his/her assignment as directed on care plans, following procedures as taught as Susquehanna Nursing Home. 2. Obtains shift to shift report from ongoing-outgoing CNA. 3. Transfers, ambulates, and transports residents as directed, using all safety measures as directed. Performs range of motion, restorative and rehabilitative procedures as instructed. 4. Completes all grooming of each assigned resident, including hair, nails, etc., as required. Insures cleanliness of environment including bed, furniture, dressers, closets and disposition of trash. 5. Responsible for taking vital signs, weights, feeding residents, and documenting findings. Also responsible for monitoring skin integrity, mobility, etc., and reporting any changes to the charge nurse. 6. Reports all incidents as they occur. Responsible for completing all daily documentation before leaving the facility at the end of the shift. Supports Golden Hill Health Care Center goal of reducing work place injuries by being compliant with safety policies. WORK ENVIRONMENT: This job requires frequent lifting, pushing, pulling and moving of over 50 pounds. The employee must walk, stoop, kneel, reach and handle, talk, hear and see. The work is inside and outside and involves a variety of duties. ACCOUNTABILITIES: Daily documentation completed by end of shift. Must treat resident information as confidential material. Follows nursing home policies and procedures. Respects residents’ rights and dignity. Reports any suspected resident abuse as required by government regulations and the nursing home policies. Attend all required inservices and seminars. Mandatory inservices must be met within time required and complete six continuing education units (CEU’s) in six months (at least one CEU per month). Required to follow established policies in regard to tardiness, attendance, and dress code. Serves as a positive ambassador to Golden Hill Heath Care Center residents, visitors and the community.
