Maintains patient confidentiality of information. Prior to initiation of dialysis, (1) performs or assures performance and charting of chlorine and chloramine test on reverse osmosis (RO) product water, (2) assures that machine has no residual disinfectant, (3) checks machine alarms, (4) assures that conductivity, temperature and pH of dialysate have been confirmed with an independent meter and charted, (5) confirms patient treatment orders for dialyzer, bath composition, pre-treatment blood work, medications or special procedures. Initiates, monitors and terminates dialysis. Performs hemodialysis treatments in accordance with approved facility policies and procedures. If mechanical problems are noted with a machine, tries to troubleshoot the problem; if unable to resolve the problem, contacts appropriate technical personnel. Recognizes changes in a patient’s condition and / or emergent situations and takes immediate and appropriate actions. Communicates patient problems to physicians and documents whether or not new orders resulted from the physician. Transcribes and implements physician orders as well as electronically enters orders in the MIS, as needed. Administers and charts all prescribed pre, intra and post dialysis medications and charge items in the Medical Information System (MIS) and on the treatment flow sheet. After dialysis, performs or assures that a machine is disinfected internally, according to the facility protocol; disinfects the outside of the machine, chair, and counter behind the machine. Disposes of contaminated trash in appropriate receptacles. Directs and monitors non-licensed patient care staff in accordance with facility policies and procedures. Documents properly the Epogen count (beginning and ending inventory count) in the Epogen overfill MIS screen. Assists in teaching and training new staff members as directed by the head nurse. Monitors patient care parameters on an ongoing basis. Identifies and coordinates patient care with physicians to comply with established protocols and local ESRD Network criteria. Documents patient care provided according to DCI and nursing standards. Collaborates with other disciplines to develop, identify and implement plans to address patient care problems. Writes a monthly progress note on every assigned patient. Participates in patient care conferences, medical rounds and chart reviews, as assigned. Provides initial and ongoing patient teaching. Documents patient education to ensure compliance with the local ESRD Network, regulatory agencies, DCI corporate and clinic requirements. Obtains medical release forms and consent forms annually, or as required. Interacts with local hospitals as a liaison to facilitate continuity of care. Works with the head nurse to maintain medication inventory of the facility. Works with the chief technician to ensure an adequate stocking of unit supplies. Reports housekeeping and equipment problems to technical staff. Knows and follows clinical policies and procedures according to accepted standards of care, DCI policy and regulatory requirements. Works with the head nurse and nurse manager to assure that occurrence reporting, fire drills, emergency preparedness reviews, mock code drills, infection control practices, OSHA compliance, sharps injury prevention programs, proper disposal of hazardous waste materials, and post exposure prophylaxis are carried out in a timely fashion. Assumed responsibility for professional growth through reading and attending conferences and workshops. Knows and follows written human resource policies. Participates in the evaluation of performance of equipment. Pursues the acquisition of required supplies and equipment through appropriate channels. Summary: Under the direction of the Hospital Services Administrator, the Hospital Services Nurse Manager is responsible for all aspects of care provided including, but not limited to, nursing staff performance and professionalism, patient and staff schedules (if program shares staff with chronic unit will collaborate with chronic manager for staff needs), and clerical\administrative needs of the program. Collaboration with physician and hospital personnel in order to enhance partnership is key to the success of the Hospital Services program. Responsibilities: Essential Duties and Responsibilities: Includes but not limited to (other duties may be assigned) the following: Supervises nursing activities to ensure that nursing care is carried out as directed, and that services are provided in accordance with physician order, DCI and hospital policy, and regulatory standards. Oversees documentation of nursing care provided, maintenance of inventory for DCI and hospital supplies, medication and dialysis bath adjustments based on current laboratory findings per physician orders, and evaluation of adequacy of dialysis. Must be attainable by pager or phone as defined by local policy to assure 24 hour access as per contract. Serves as back up to staff (required to review and sign job description for any role performed). If Hospital Services program consists of multiple facilities, adequate transportation is necessary to be able to maneuver between locations as needed. Collaborates with Chief Technician concerning repair and maintenance of all equipment and water treatment practices per industry standards. Serves as the primary contact to the Medical Director. Able to openly communicate with all nephrologists regarding the services provided. Serves as DCI liaison with Hospital Administration, in collaboration with DCI Local Administrator. Assigns duties to Hospital Services staff, and prepares on call and dialysis schedules that ensure continuous coverage as per Hospital Services contract. (Coordinates staffing needs with chronic Nurse Manager if staff shared). Completes performance evaluations on all staff at the completion of the probationary period and at least annually thereafter. (Provides input to chronic Nurse Manager for shared staff). Recommends and implements employee discipline (after discussion with Administrator and DCI Human Resources as appropriate). Interviews and recommends hiring for applicants to the Hospital Services Program. Supervises the orientation of all staff once they are onsite at the DCI Hospital Services program. Collaborates with the DCI Educator for training as outlined by Hospital Services Education Plan (approved by Governing Body) and set forth in local policy by Administrator. Acts as a mentor to the Hospital Services staff. Evaluates patient care delivered and makes recommendations to physicians as appropriate. Collaborates with hospital discharge planners and/or case managers and outpatient units to ensure smooth transfer to the next level of care. Initiates, reviews and revises Hospital Services Policies and Procedures. Facilitates approval by DCI Governing Body before implementation. Acts as the Medical Record Manager for treatment records of hospitalized patients which would include audits of care given. Accountable for nursing budget/expenses, and makes recommendations regarding equipment and supplies. Maintains documentation of treatments and provides required information to ensure that DCI Hospital Services invoices accurately reflect services provided per contract. Participates in education of DCI staff, patients, and hospital staff. Maintains working knowledge of industry standards related to renal replacement therapy in the hospital setting, including ESRD and AKI concepts/principles, regulatory, and accreditation standards. Educates staff regarding industry changes and updates. Facilitates staff meetings (with minutes) to keep staff up to date on program, hospital, industry, regulatory, and accreditation standards. Develops process for holding staff not attending meetings accountable for material presented. Assures that staff receives all mandatory education required. Assures that Hospital Services staff personnel files are current to include licensure, required certifications, initial and annual competency verification, and required health records (i.e. Hepatitis and TB status). Recognizes and responds appropriately to difficult patient/family situations and emergency situations. Maintains a line of communication with appropriate hospital departments, DCI clinical staff and DCI Management. Actively participates in the DCI Risk Management and QAPI programs, including timely completion of required audits and regular attendance at DCI QAPI and Governing Body meetings.
