May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury, in accordance with Organization policy. May fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function. May train patient in the use of prosthetic and/or orthotic devices. Maintains appropriate clinical records, clinical notes, and reports to the physician any changes in the patient’s condition. Submits these records in accordance with Organization policy. Maintains contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care. Attends and contributes to in-services, case conferences, and other meetings as required by Organization policy to ensure coordinated and comprehensive plans of care for the patients of the Organization. Identifies patient and family/caregiver needs for other home health services and refers as appropriate. Instructs patient’s family/caregiver and other Organization health care personnel in patient’s treatment regime when indicated and appropriate.

Performance Expectations: Is knowledgeable of current industry standards and maintains knowledge base to provide leadership Demonstrates a commitment to continuous quality improvement and provides education and support to improve outcomes throughout the organization Implements plans to meet operational goals of facility Incorporates CDC Cares into daily practice Communicates professionally with staff Maintains confidentiality and professionalism in al interactions Collaborates with Facility Manager in the evaluation process of all direct care staff Acts as a resource for staff Responsible for the oversight, direction, and evaluation of all direct care staff that provide dialysis and nursing care in the facility Contributes to the hiring and terminating of direct care staff Contributes to the facility’s QAPI program Functions as a staff RN as assigned Quality Management Responsibilities: Participates in and completes components of facility’s QAPI program Trains staff on CDC quality program, standards, expectations and methods to improve results Routinely reviews quality outcomes and works with staff to achieve CDC quality outcome goals Provides leadership in implanting quality improvement initiatives in the facility Supports the Facility Manager in assuring facility compliance with Medicare Conditions for Coverage Ensures program is compliant and ready for licensure and certification survey process Implements the Plans of Correction for licensure and/or certification surveys Oversees medical record audits as directed Reviews and provides input for revision of patient care policies as directed Assists in the leadership of facility quality teams Operational Management Responsibilities: Assists with the implementation of patient care policies that are new or revised Collaborates on solutions to address budget variances as needed Implements product and policy evaluation trials on site as directed Monitors adherence with CDC policy and procedure Responsible for holding monthly unit staff meetings Is on-call for emergency issues at the facility as needed Collaborates with Facility Manager in the creation and maintenance of systems that facilitate smooth operation in the facility Assists in the planning and implantation of plans to promote facility growth Keeps Facility Manager informed in a timely basis of unit activities Coordinates staffing resources with other Facility Managers and Director of Clinical Operations Patient Care Responsibilities: Functions in staffing according to schedule and patient load demands as needed Ensures all CIA/RIA/POC are completed according to CDC policy Facilitates intake of new patients utilizing CDC’s centralized admission process Develops and maintains professional relationships with referral sources and Medical Director Schedules and assures completion of new patient orientation per policy Monitors and assures patient education is completed Implements suggested dose guidelines and protocol dosing requirements for designated medications Reviews monthly labs to ensure completion and identified issues are addressed Enters in RM Pro patient complaints and concerns and follows up on tasks assigned Ensures all policy and procedure is consistently followed Safety Responsibilities: Participates in the investigation of employee injuries to prevent and correct safety hazards Assures all equipment and safety guidelines are adhered to in all operations Assures new safety policies and procedures are fully implanted per guidelines Compliance Responsibilities: Demonstrates knowledge of CDC Code of Ethics and Business Conduct and Compliance plan Implements plans to assure facility compliance with CMS guidlines
