Transitional Care Lead (LPN or Social Worker)

Details: Transitional Care Leader is responsible for overseeing and facilitating all transitional care activities from initial hospital admission to the conclusion of the episode, typically 90 days post-discharge. TCL will work exclusively with the Episode Initiator, New England Inpatient Specialists, across three inpatient locations. Transitional care activities include patient assessment and education, discharge planning, development of a post-acute care plan, arrangement and coordination of post-acute services, and direct follow-up on patients’ progress relative to their post-acute plan. Success in the role will be defined by improved patient outcomes and experience of care as well as improved utilization of post-acute resources. II. ESSENTIAL DUTIES AND RESPONSIBILITES: Initiate prospective patient identification and confirmation of bundled care patients; confirm data is entered in Episode Connect (EC) Collaborate with inpatient facilities to ensure ‘working DRG’ is established within 24 hours Onboard patient: Complete initial patient meeting within 24 hours of admission; provide copy of BPCI brochure and Medicare Letter (compliance required) to patient/family. Complete patient assessment to determine the appropriate level of postacute care and mitigate readmission risk. Develop inpatient and transitional care plan in collaboration with New England Inpatient Specialist Physicians and hospital case management. Work with New England Inpatient Specialists Physicians and hospital’s case management department to ensure safe discharge to appropriate next site of care; including assessment and coordination of post-discharge needs (e.g., DME). Establish contact with the Next Site of Care (NSOC) provider prior to the patient’s discharge and implement plan to ensure contact/follow up through the 90 day Post-Acute Care (PAC) phase. When appropriate collaborate with New England Community Medical Services to identify patients discharging to post acute facility serviced by NECMS. Facilitate key clinical information transfer between acute and postacute providers. 10. Facilitate scheduling of effective transitional care services. 11. Follow-up on transitional care plan progress with patient and post-acute providers. Provide assessment of readmissions to determine cause and develop mitigation plan. 12. Facilitate transitional care management meetings with post-acute providers. 13. Review TCS process and outcomes performance to drive performance. 14. Develop working relationships with hospital interdisciplinary team members including, but not limited to Case Managers, Discharge planners, Social Workers, Coding Supervisors etc 15. Update and monitor Episode Connect daily; identify high risk patients and ensure appropriate documentation/follow up. 16.
