Summary - RN Under the general supervision of the RN Manager, she/he provides intermittent skilled nursing services; communicates the patient’s progress with other disciplines and directs, supervises and instructs non-professional home health aide staff in the provision of personal care to the patient. . Responsibilities – RN: Under the physician’s order, admits patients eligible for hospice services. Develops patient care plan that specifically addresses identified patient problems; nursing problems and goals. Updates care plans on an ongoing basis; revises and resolves patient problems and goals as changes occur and/or recertification.. Provides intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities as outlined in the patient Plan of Treatment. Submits completed skilled nursing notes; communication notes and home health aide supervisory notes per policy. Submits recertification paperwork by the due date provided by the RN Manager. Effectively communicates with all members of the healthcare team. Acts as the patient’s advocate and as such are a liaison to assist in communicating the patient’s needs to the multidisciplinary team. Supervises the home health aide every 14 days. Provides direction and instruction as it relates to provision of personal care and related support services. Participates in PI program through submission of data collection as it relates to direct patient care problems and serving on PI teams. Participates in discharge planning process. PERFORMS CLERICAL DUTIES RELATED TO PROCEDURES, AS THEY RELATE TO THE PATIENT CARE UNIT. 2. Enters physician orders in a timely manner and with accuracy, prioritizing order entry to ensure quality patient care. 3. Ensures that all charts are supplied with appropriate forms for documentation. 4. All reports are posted in a timely and accurate manner. 5. Assists physicians, nurses and other members of the health care team with clerical duties, retrieving reports, etc. 6. Relays messages to appropriate caregiver within 5 minutes of receiving message. Adheres to call bell response protocol. 7. Ensures that diet/tube feeding changes are completed and credited every shift. 8. Checks food trays within 10 minutes of arrival to floor. Cleans refrigerator, microwave and other areas when indicated 14. Maintains the integrity of the Pyxis Machine and inventory through pushing the “take” button, 15. Protects the environment of the patient care area as well as the hospital campus from vandalism and litter. 16. Consistently uses appropriate “scripting’ as instructed when communicating with patients and their families. 17. Expense Management - Controls operating expenses by monitoring employee productivity through use of the distribution system, service scheduling calendar, etc... in order to manage and optimize personnel and vehicle expenses; managing District parts and fittings inventory to reduce repair and maintenance expense; enforcing credit and collections policies and procedures to reduce DSO’s, improve aging results, and reduce write offs; and proactive daily involvement in all District purchasing and expenditures. Administrative Management - Ensures all District daily, weekly and monthly work processes and reporting are done accurately and timely. Other - Performs delivery, service, sales, customer service, and/or District administrative work as required. SUPERVISORY RESPONSIBILITIES Directly supervises up to 30 employees in the District. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.
