Gathers, organizes, documents (in CPR+ and electronic medical record), and reports clinical data for review and action by the patient care team (Pharmacist, Nurse, prescriber) to include: 1. lab data; 2. b. results of patient contact documented on the custom assessment; 3. c. hospitalizations; 4. d. follow-up appointments. 11. Generates pharmacy and clinical reports from CPR+ as directed. 12. Coordinates with warehouse and pharmacy team to ensure all medications needed are available for patient care. 13. Completes all necessary paperwork, forms, and records according to policy. 14. Participates in the pharmacy on-call rotation. 15. Maintains solid knowledge of, and compliance with, Company policies and procedures. 16.
