Case Manager/Coaches

Details: JOB SUMMARY This position is responsible for providing quality services and intensive Bi-Level Case Management; managing the process of service delivery from enrollment to goal achievement to youth offenders between the ages of 14-24 years old ensuring that quality outcomes are achieved through advocacy and well establish rapport with the people serve. Also requires the ability to maintain documentation and perform job related duties as prescribed in job related duties. ESSENTIAL DUTIES AND RESPONSIBILITIES Assist participants to develop and coordinate Individual Program Plan and Individual Career Plan. Documents services and ensures that records are completed according to established policies and procedures. Implements initial informational gathering and assist participants in assessing and evaluating goals and or training goals for their Individual Program and Career Plans. Coach and encourage youth participation through employment, education, and career progression. Counsels participant on personal, financial, employment behavioral, crisis, substance abuse, educational and training matters. Conducts on site visits to correctional facilities, detention centers, participant’s home and work locations for monitoring and support to record their progress. Help participant build positive social networks through service learning projects. Work closely with corrections officer and courts to advocate and provide support to participant within due process. Must have general knowledge of the criminal justice system. Be able to assist participants with completing documents as it relates to judiciary procedures such as expungements and diversion program. Participate in program related trainings and other training programs as required. Keep supervisory staff apprised of gaps in services available and assist in services needed for client’s success. Prepare required reports, surveys and documentation as required by funders and program. Cultivate and sustain professional relationships with the frontline staff of partner agencies. Maintain caseload with accuracy and professionalism. Be able to assess and document needs through one-on-one meetings with participant. Must have the ability to handle multiple priorities simultaneously. Must be able to establish and develop partnerships with local community agencies. Recruit mentors to match with participants. Perform other duties as assigned. KNOWLEDGE, SKILLS AND ABILITIES This position prefers the incumbent to have completed education at the Bachelor’s degree level. Preferably in Human Services or Criminal Justice and two (2) years of direct service experience. The ability to communicate with individuals from diverse backgrounds and the ability to adapt to working effectively in high stress and fast paced environments. Proficient computer skills are required including Microsoft Office, Power Point Presentation and the Internet. Demonstrated knowledge of the criminal justice system, human behavior, and social work skills is preferred. Incumbent must have the ability to work with individuals with a criminal history. Must be knowledgeable of community resources; develop an understanding of the local employment market, knowledgeable of available education, training and vocational resources. PHYSICAL REQUIREMENTS The employee may be required to stand, bend, lift and stoop in performing work. SPECIAL REQUIREMENTS Must be able to pass a National Criminal Identification Center clearance. May have to work outside of normal working hours; must have a valid driver’s license and have access to a registered, insured vehicle.
