The Clinical Social Worker for the Liver and Pancreas Center (LPC) provides a wide array of clinical services with patients and their families, as well as psychosocial and resource consultation to UW Health staff . This position acts as a resource for nursing and physician staff. As part of this process, the Clinical Social Worker - LPC monitors patients’ health and psychosocial needs across the total health care continuum—from the community, the outpatient clinic, the inpatient unit and back to the community. Problems encountered have a significant level of complexity due to both clinical and logistical issues. Most problems can be resolved by referring to subject matter experts, program director, or references. A considerable amount of analysis and innovation is involved in problem solving and applying appropriate interventions and therapies with individual patients/families and patient problems. A wide variety of internal and external relationships are involved in performance of the LPC social worker’s duties. In the clinics and hospital, there are patients, families, nurses, pharmacists, other social workers, financial counselors, and physicians (both faculty and house staff). Outside the institution, there is ongoing involvement with community physicians, county social workers, drug and alcohol counselors, pharmacists, laboratory personnel, pharmaceutical representatives, insurance company case managers and medical directors. Organizational skill is paramount. In addition to independently planning and scheduling his/her own daily schedule, the Clinical Social Worker - LPC must be able to prioritize clinical issues and case manage a large number of patients/families in various stages of their liver disease. Knowledge of computer software is required to operate within the program’s local area network—entering and obtaining information in patient problem lists, medication and lab profiles, and clinical research database—as well as UWHC information system. The Clinical Social Worker - LPC must demonstrate competence and management in the provision of an array of psychosocial services to patients, their families and supports, across clinical settings. It is expected that this position would be available to respond to a variety of needs from financial and insurance coverage issues affecting patients, to identifying emotional and crisis response to situations that may arise. This position has a dual reporting relationship: primary to the Manager of Liver and Pancreas Transplant and secondary for clinical oversight to the Social Work Manager. Clinical Social Worker (Part-Time/Overnight) - Emergency Department

The Emergency Department (ED) Social Worker is assigned to the UWHC Emergency Department and serves as an expert in the area of crisis and trauma interventions for ED patients and families. The ED social worker works closely with the trauma teams, assisting the team with identification of the patient, contacting and providing support to the patient’s family. This social worker is a part of the ED interdisciplinary team receiving consultation requests from the other ED disciplines and also doing case finding as appropriate and as time allows. The focus of services provided by the ED social worker is that of crisis and trauma interventions. She/he provides immediate grief counseling during traumatic events and is available to assist the trauma team in communicating with the patient’s family/support system. In addition the ED social worker works with patients and families performing psychosocial assessment, resource planning, assisting with financial/legal issues, facility placements, and education to patient/family. The ED social worker routinely develops safe and effective discharge plans from the ED that may include referrals to mental health and domestic/elder/child abuse resources in Dane County and the surrounding areas. The social worker plays a key role in the reporting of concerns to the appropriate authorities as well as assisting with the coordination of the systems involved in the investigation and disposition for the patient. The social worker coordinates referrals to both internal and external providers. The ED social worker must have the ability to respond to and prioritize consultation requests from physicians, other hospital disciplines and community professionals. Essential skills for this position include: resource management, knowledge of financial and community resources, knowledge of advance medical directives, knowledge of guardianship processes, and the ability to assess patient needs and work with the patient and the patient’s family to formulate, understand and accept the plan for care. The social worker utilizes brief interventions to enhance patient functioning and emotional well being. This position carries out all responsibilities in accordance with the organization's core values, applicable policies, procedures, and civil laws. He/she must also be able to work autonomously, maintain patient confidentiality, and to utilize organizational skills essential to successfully provide the diverse services which patients and families require. Details: UHS is hiring for a Hospital Service Technician to work PT evenings and weekends in Madison, WI!!!! The Hospital Service Technician is responsible for equipment management tasks in the hospital, including: delivering equipment, accessories and disposables to patient rooms; delivering equipment to standby locations; picking up soiled equipment; cleaning and processing equipment; conducting equipment tracking rounds throughout the patient rooms; and recording each activity by scanning equipment as it is completed. Knowledge and Physical Requirements High school diploma or equivalent. Prior work experience in hospital setting or customer service preferred. Basic computer skills. Willing to work flexible hours, including evenings, weekends and holidays, as well as emergency off-hours as required. Able to lift and/or push 75 pounds. Records all non-functional equipment that has been reported by customer. Completes safety inspections/preventative maintenance on equipment (when applicable). Documents all equipment incidents in occurrence with both UHS and customer policies. Provides all customer feedback to UHS supervisor in a timely manner. Performs other assigned duties. Development Level II Proficient in the UHS systems and Microsoft Office programs (Word, Excel). Serves as part of training team for other members in Division/Region. Serves as a peer advisor to employees. Orients, trains and assigns and checks other employees' work, as appropriate. Certified to complete safety inspections on core equipment within the account. Possesses strong customer interfacing skills based on relationships established within accounts.
