Details: Job Summary : Provides, oversees, and/or administers a wide range of psychosocial evaluations, assessments, counseling, crisis intervention and /or care management services. Requires a high degree of independent decision-making that is congruent with Alexian Brothers Community Services (ABCS) mission and values. Adheres to the National Association of Social Work Code of Ethics, and is responsive to the ethical and legal issues in participant care. Duties and Responsibilities : Participates as an active member of the Interdisciplinary Team (IDT) and provides ongoing monitoring of participants care in assigned caseload. Conducts initial and scheduled assessments through interview, psycho-social tests, observation techniques, etc. Evaluates information from assessments to identify cause of problems/issues and recommends appropriate plan of action or referral to other disciplines. Assists participant/caregiver in completing relevant forms during initial and scheduled assessments, i.e. Advance Directives, Durable Power of Attorney (DPOA), Emergency Care Plan (ERCP), etc. Regularly reviews/updates forms with participant/caregiver at scheduled reassessments or more often if necessary. At scheduled reassessments and as necessary, reviews/updates participant contact information on participant face sheet. Follows ABCS policy/practice to make changes to face sheet as necessary. Counsels participant//DPOA/caregiver in decision-making related to end of life care planning. Specifically addressing funeral planning/preparation. Establishes and maintains effective working relationships with participants/caregivers in caseload. At scheduled reassessment, or more often if necessary, assesses caregiver stress level and makes intervention recommendations to the IDT. Coordinates and facilitates participant/caregiver meetings. Designs appropriate intervention and discussion points and executes during the course of the meeting. Identifies a course of action, recommends/reviews with IDT prior to final dissemination to participant/caregiver. Provides and/or arranges for interventions as appropriate for participants/caregivers in a crisis situation. Establishes and maintains current, written case management records, including ongoing documentation of services provided/offered and referrals made on behalf of participants. Provides counseling sessions for participants/caregivers, makes home visits for scheduled participant reassessments. Refers participants/caregivers to appropriate social service agencies for financial assistance and other services not directly provided by ABCS i.e. subsidized housing, utility assistance, rent assistance, etc. Coordinates admissions to nursing home. Facilitates patient care activities, including revision of direct patient care and collaborates with multidisciplinary team members to achieve quality patient driven outcomes. Requirements: Graduated from an accredited School of Nursing and must have current Illinois RN License. BSN preferred. BLS and ACLS are required. Excellent customer service skills, time management/organization skills and commitment. Certification in specialty area preferred. Previous experience in Med/Surg and/or Telemetry is preferred. Bilingual English/Spanish or English/Polish preferred.
