Transitional Care Coordinator

Details: Goal: To monitor and reduce 30 day readmissions for Skilled Nursing Facilities. Accurately identify gaps in care and communicate findings to the appropriate personnel/departments. -All post-discharge follow up appointments are scheduled and patients are made aware of such appointments (PCP, Specialist, Clinics, etc.). -Assist with scheduling of transportation (if necessary). -Assist with post discharge medications (orders are filled; med regiment is understood/reviewed by patient). -Make sure home care is timely on visits and patient is satisfied with service being delivered. -Make sure DME has been ordered and delivered on time. -Make home visits if necessary. -Assist with avoidable readmissions (transition patient to next/prior level of care if necessary… i.e. readmit to SNF, consult Hospice, etc.). -TCN will follow patients 48 hours after d/c followed by Day 7, Day 15, and Day 30, or as requested/needed by patient.
