Provide Case Management services to referred pediatric members and recently discharged NICU members: • Act as the single point of contact/liaison to facilities, providers and/or members. • Complete member surveys, assessments and care plans • Request and review medical records • Provide community resources and referrals • Provide education on disease processes • Collaborate with the interdisciplinary team to incorporate best practices, assess outcomes and develop the individualized care plans • Mail educational resources on disease conditions to members • Review referrals and the need for Speech Therapy/Physical Therapy/Occupational Therapy • Identify CRS diagnosis within pediatric population through Utilization Review/Case Management Departments • Review denial/acceptance letters received from CRS Provide admission and concurrent review on inpatient members to ensure and facilitate quality and cost effective health care delivery: • Review all inpatient admissions and initiates discharge planning in a timely manner • Utilize clinical skills, chart review, physician communication and Interqual Level of Care Criteria • Conduct ongoing review for members in inpatient stay and utilize peer review, as necessary • Complete onsite and/or telephonic admission and concurrent reviews for medical necessity • Communicate requests which do not meet criteria with UR Coordinator and Health Choice Medical Director(s) for determination • Provide notification to facility of determination • Facilitate member’s transfers within contracted facilities for ongoing inpatient stays • Initiate interdepartmental coordination to ensure quality and timely care for members • Complete prior authorizations as needed for discharge planning • Monitor utilization of non-medically necessary services and redirect as needed
