Details: Job Summary: The RN Care Coordinator is responsible for assessing, planning, facilitating and advocating for options and services through a continuum of care from point of contact through discharge on assigned patients. The RN Care Coordinator performs this role in such a manner as to meet the individuals health needs while promoting quality and cost effective outcomes. The positions emphasis will be on care coordination, communication and collaboration with nursing, physicians, departments within the medical center, insurers and post-acute service providers to pace the care toward optimal outcomes within the appropriate level of care. The RN Care Coordinator will advocate for the patient and family by identifying and valuing patient choice, spiritual needs, cultural, language and socioeconomic barriers to care transitions. In addition, the RN Care Coordinator will protect confidentiality while striving to achieve high levels of patient satisfaction. The RN Care Coordinator consistently conducts the utilization review process and the discharge planning process as required by hospital policies, standards of practice and Federal and State regulations. This position is represented by CNA and is covered by the terms and conditions of the applicable collective bargaining agreement. Require Patient Population tab: No Experience Minimum - Minimum of 3 years nursing experience in an acute hospital setting. Prior Care Coordination experience in a clinical or insurance setting is required. If operational conditions permit, training a candidate without the required experience may be considered. Works collaboratively with the child, family, Wraparound Facilitator, Wraparound Child and Family Specialist, Wraparound Therapist, identified DCFS and/or Department of Probation personnel, school staff, and all other inter and intra-agency personnel in order to provide advocacy and support services, parent education and information, and identified Targeted Case Management (TCM) and Crisis Intervention services in accordance with the POC and/or Client Treatment Plan (CTP). Facilitates the linkage of clients and/or family to identified and needed community services, inclusive of (but not limited to) initiating contact with community resource personnel, providing the respective information to the client and/or family, accompanying the client and/or family to intake/screening appointments, consulting with resource personnel, and taking the steps necessary to ensure that reasonable efforts have been made towards accessing needed services/resources. Leads/Co-leads parent groups (support and/or educational) for current and discharged families. Able to establish and maintain appropriate rapport with children, adolescents, families, inter and intra-agency personnel, and regulatory bodies. Provides peer support to parents served by the Wraparound Program and assists them to successfully engage with the process and staff. Provides 24-hour availability and crisis intervention on an as needed basis or regularly scheduled basis which may include weekends and evenings.
