Details: The duties of the Social Worker include providing a medically-related social service program to meet the needs of the residents. This includes: completing PASARR screens, orienting the residents and families to Oak Park Place, completing the required admission paperwork, completing a detailed social history, admission note, psychotropic consents, helping the residents and families adjust to the center, completion of MDS, RAPS, care plans, cognitive assessments, depression assessments, quarterly and annual assessments and updating care plans prn for purposes of identifying problems and formulating resolutions. Comprehensive discharge planning is required including referrals to agencies and utilizing resources in the area to promote a safe and successful return into the community. Assisting families and residents with financial questions and referrals such as, but not limited to, Medicaid, Social Security, Medicare, Veterans Benefits, and insurance. Ongoing reviews with patients on psychotropic medication including promoting reduction trials as required. All of the above is in accordance with federal, state and local regulations and the current established policies and procedures of the facility.
