RESPONSIBILITIES* Identifies and collects information concerning the health status of patients.o Distinguishes between abnormal and normal findings.o Reports abnormal findings to the RN.* Assists in determining patient problems, age-based needs and nursing interventions.* Implements interventions defined in the plan of care as directed by the Charge Nurse.* Provides patient education.* Reinforces patient education as necessary.* Contributes to the evaluation of the patient's responses to interventions and to the revision of the plan of care as needed.* Reviews physician's orders and collaborates with R.N. and other healthcare team members regarding implementation of the plan of care. Participates in quality improvement efforts in the department and/or hospital. Collects quality management data as directed. Practices principles of risk management, safety and infection control Reports non-compliance to accepted standards of care to the RN. Demonstrates accountability for practice. - Provides input to solve departmental problems and improve patient care. - Participates on committees as requested. - Practices within legal and regulatory parameters. - Provides input to the assessment of learning needs of team members. Uses material resources effectively and efficiently. Organizes work flow to allow for changes in patient status. Plans, organizes and evaluates material resources for effective, efficient use specific to assignment. Models behavior reflective of the purpose, philosophy and policies of Patient Care Services and Columbia Care Center. Maintains confidentiality of patient and employee information and records. Maintains customer relations to include patients, families, physicians, co-workers and other departments. Maintains the image of Columbia Care Center and work area as a safe, clean, efficient, caring organization by demonstrating a team approach which produces an environment conducive to patient, family and team well-being.
