Maintains established hospital and departmental policies and procedures, objectives, customer service guidelines, performance improvement program, safety, environmental, and infection control standards. Documents evaluations, treatment goals, and plans within 24 hours of completion. Regularly updates documentation and maintains all required prescriptions and signatures as required by state, federal, and payer guidelines. Completes timely discharge summaries within one month of their last appointment. Enhances professional growth and development of self and others through participation in educational programs, current literature, in-service meetings, and workshops. Provides direction and leadership to staff, students and new graduates. Acts as a resource to promote physical therapy within the community. Ensures that all physicians are credentialed and current information is on file for all attending, admitting and staff physicians. Member of the Governing Body. Receives and documents physician orders and provides notification of physician regarding patient status changes and needs when necessary. Receives new patient authorizations and proceeds with obtaining new patient orders, performs assessments and documents initiation of the medical record. Follows up on complaints from employees, staff, patients and customers, perform investigation and determine with governing body/QAPI committee what plan of action to take. Monitors variance/incident reports and devise plan of action to the QAPI committee monthly. Formulates ESRD death reviews for each death and submit annual report to QAPI committee. Performs other duties as assigned. Requirements: Graduation from accredited school of nursing, B.S. in Nursing preferred. R.N. with valid licensure in the applicable state. At least three (3) years of experience in dialysis nursing, or has at least 2 years dialysis experience and holds a current certification from a nationally recognized Board in Nephrology nursing or hemodialysis experience in the care of ESRD clients.
