Details: Valley Mountain Regional Center is a non-profit agency that serves children and adults with developmental disabilities in San Joaquin, Stanislaus, Amador, Calaveras and Tuolumne counties. We are currently seeking Service Coordinators to manage our client cases and coordinate services in our Stockton, Modesto, and San Andreas offices. General Statement of Duties : Under direction of the Program Manager, the Service Coordinator will participate in the assessment of the needs of consumers who are developmentally disabled. As facilitator of the Interdisciplinary (ID) team, the Service Coordinator will develop, implement, and coordinate a program plan for each consumer. The Service Coordinator will project and advocate the legal, civil, and service rights of consumers who are developmentally disabled, and perform related work as required. Key Responsibilities Assess, monitor, coordinate and implement IPP/IFSP through the ID Team process. Schedule and facilitate annual reviews, quarterly reviews, wellness review examinations, clinical consultations, and ID/IFSP Team meetings as required. Completion of case documentation and procedures, including Confidential Summary, Purchase of Service (POS), Individual Program Plans (IPP), Consumer Development Evaluation Reports (CDER), Quarterly Progress Reports (QPR), Facility Liaison Consumer QA reports, Life Quality Assessments, Title XIX, court reports, Medicaid Waiver, and other correspondence and documentation as required. Provide advocacy, education, crisis, intervention, and other support for families and consumers. Serve as liaison to care homes, day programs, supported living agencies, schools, and other community agencies or service providers as designated. Collect, synthesize, and report consumer statistical data as requested, including state surveys, internal surveys, and community agency surveys. Participate on internal and external committees for purposes including public education, improving internal agency functions and promoting positive communication with external agencies. Participate in in-service and out-service training to increase knowledge and skills in the field of developmental services and other related areas. Serve as Officer of the Day (OD) and on After Hours Response System (AHRS) as assigned. AHRS is voluntary. Complete all required non-consumer related documentation, including travel forms, time sheets, monthly activity forms, and other forms. Perform other non-management duties as assigned.

Provision of specialized clinical services and/or program development activities providing these duties have been assumed with the mutual agreement of Director of Mental Health Services and/or designee. Crisis intervention can include consultation with institutional staff regarding management concerns, referral, and/or follow-up. Crisis intervention activities may require on-call services on a rotating basis. Provision of psychological evaluations as requested by appropriate agencies in accordance with the policy and procedure of Mental Health Services. Evaluations are to be completed in a timely manner and include the administration and interpretation of psychological assessment techniques appropriate to referral and within the scope of staff member's education and professional experience. Provision of training in human behavior and/or mental health issues to institutional staff in accordance with institutions' Training Program and as assigned by the Coordinator of Mental Health. Provision of documentation of individual patient interventions and staff member's clinical activities in accordance with the policies and procedures of Mental Health Services. Participation in staff meetings and in-service training programs as offered by Mental Health Services. Inform Coordinator of Mental Health Services or Director of Mental Health Services of personal need for additional clinical supervision, overall problems in delivery of clinical services and/or proposals to improve clinical services. Provision of assistance to staff Psychiatrist in monitoring of patients receiving psychotropic medications. Assist in effecting transfer to inpatient psychiatric setting when inmate's mental status indicates he/she is a danger to self or others. Assist in coordinating aftercare referral when "special needs" inmates are release from facility. When requested, will complete an intake mental health evaluation, including mental status and psychosocial information to ensure patients are provided appropriate mental health followup. Compliance with employee standards of Corizon and applicable employee standards of the contract. Compliance includes, but is not limited to, the maintenance of a working environment and personal appearance consistent with professional responsibilities, development of harmonious working relationships and timely notification of supervisory personnel of absences from institution.

Tests and inspects installed machinery and equipment. May also set up and operate such machine shop tools as lathe, boring mill, planer, shaper, slotter, and milling machine to fabricate replacement parts. Repairs and maintains diesel engines used to power ships, using hand tools, precision measuring instruments, and machine tools Diagnoses trouble, disassembles engines, and examines parts for defects and excessive wear. Reconditions and replaces parts, for example pistons, bearings, gears, valves, and bushings, using engine lathes, boring machines, hand tools, and precision measuring instruments. Positions, aligns, fits, and welds parts to form complete units or sub-units, following blueprints and layout specifications, and using jigs, welding torches, and hand tools. Monitor engine, machinery, or equipment indicators Maintain electrical power, heating, ventilation, refrigeration, water, or sewerage systems. Note gauge readings or test data, such as revolutions per minute or voltage output, in engineering logs Start engines to propel ships and regulate engines and power transmissions to control speeds of ships, monitor and test operations of engines or other equipment so that malfunctions and their causes can be identified. Maintain or repair engines, electric motors, pumps, winches, or other mechanical or electrical equipment or assist other crew members with maintenance or repair duties. Perform general marine vessel maintenance or repair work, such as repairing leaks, finishing interiors, refueling, or maintaining decks. Operate safety equipment and use safe work habits.
