Nurse Case Manager (RN Registered Nurse) Job Responsibilities As a Nurse Case Manager, you will direct and supervise all personnel involved in patient care within a specific geographic area. This involves making initial patient evaluation visits and consulting with the attending physician to establish and update the plan of care. You will be responsible for ensuring quality patient care and the allocation of agency resources. Additional responsibilities: •Making initial client evaluation and assessment visits. Consulting with client, family members and office staff in establishing, reviewing and updating the plan of care and services as necessary •Reviewing new case referral and available client information related to the case, including need for disciplines that may be required to meet the client’s home care and service needs. Communicating client information to the Clinical Manager and Client Services Manager •Assigning patients to Nurses and other members of the health team in conjunction with the Clinical Manager •Consulting with the Clinical Manager about problems concerning the admission of patients to Agency care and coordination of services •Assisting the Clinical Manager in the supervision of home health aides, certified nurse’s aides, nurses and/or other ancillary service personnel •Reviewing plans for patient treatment and notifying team members of any changes •Participating in the nurse on-call system •Participating in staff development activities, meetings, and training •Initiating and conducting Care Team Conferences for patients •Completing assessments, re-assessments and client visits and providing related supervision in a timely manner. •Determining the need for additional supervisory assessments and providing the updated documentation in the client’s home in a timely manner. •Reviewing medical records for assigned case load and assuring that the client’s care and service record clearly outlines the reason for admission for care and services, the status of the patient throughout service, the delivery of care and services according to the plan of care and physician’s orders (if applicable) and recording the client’s responses to care and services provided, including satisfaction with care •Ensuring that all records are in compliance with agency policies and procedures •Following all privacy policies of CareSouth and maintaining the confidentiality of protected healthcare information (PHI) Nurse Case Manager (RN Registered Nurse)
