Patient Services Managers (#PSM-C) have primary responsibility for ensuring that clients receive necessary services in a timely manner by supervising the RNs providing care and performing quality and utilization reviews of all clinical documentation. Qualifications : BSN with three (3) years of nursing experience, one of which must be in a CHHA operation. HEALTH PLAN OPERATION : Assistant Director of Quality Improvement (#ADQI-CM) is an office based position that systematically monitors, evaluates and improves the quality and appropriateness of care and services that are either provided or coordinated for our members and to maximize member and plan satisfaction. Qualifications: BSN preferred with three (3) years of RN clinical experience, one of which must be in home care. Quality improvement experience is mandatory and must be UAS Certified. RN Care Manager Trainer (#RNCMT-CM) is an office based position that is responsible for the development and presentation of all clinical training for new hires and to provide continuing education to existing care management staff. Qualifications: RN (BS preferred). Home Care experience with knowledge of care management, training and education and one (1) year of hospital experience required. UAS certified. UAS RN (#UAS-CM ) is a field based position for the purpose of conducting MLTC intake and re-assessment visits. The UAS RN takes independent ownership for self-scheduling visits to determine member service needs and continued eligibility. The UAS Nurse coordinates with the care management team or MLTC Contract to maintain a high level and effective individualized care management operation. Qualifications: RN (BSN preferred). One (1) year of home health care with working knowledge of Medicaid Managed Long Term Care plans and Medicare and Medicaid rules and regulations. Must be UAS certified. Bilingual Spanish and Asian preferred RN Care Managers (#RN-CM) is an office based position that is responsible for the planning and coordination of care management for community based participants. The RN will collaborate with the LMSW, Service Coordinator and other members of the care management team to monitor effectiveness of the care plan and to promote participant/care giver satisfaction with services. Qualifications : RN (BSN preferred). Experience as a NY licensed RN working in a home care environment or as a staff nurse in an acute, sub-acute, or long term care (LTC) setting or managed long term care plan (MLTCP). Bilingual Spanish and Asian and UAS certification is preferred. Ability to accurately assess client health care status and psycho social needs as well as ability to establish a Plan of Care which will assist the client to reach desirable health status. Social Workers (LMSW) (#SW-CM) promote and maintain mental and social health of enrolled members through assessment, treatment, teaching and counseling. Responsible for the implementation of a social work care plan and coordination of social work with other services and maintain active membership with MLTC committee groups to support and facilitate CQI, client satisfaction. Qualifications: LMSW. One (1) year of work experience in a home care operation with a working knowledge of current community health practices for the frail elderly from direct service experience, and bilingual Spanish and Asian preferred. VISITING NURSE PRACTITIONER PROGRAM: Nurse Practitioner (NP) (#NP-VNPP) performs comprehensive assessments on clients to diagnose, develop, implement and coordinate a treatment plan for ailments and/or injuries as well as to teach and promote health maintenance and refer clients to appropriate licensed professionals. Qualifications: Licensed Nurse Practitioner from an accredited institution of medicine. Experience working in a home care environment or as a staff nurse in an acute, sub-acute, or long term care (LTC) setting or managed long term care plan (MLTCP).
